Miscarriage occurs in up to 1 in 4 pregnancies and is rarely caused by anything a woman did or did not do. Regardless of gestation, miscarriage is both a medical and emotional experience.

Diagnosis

  • Ultrasound confirmation of early pregnancy loss

  • Hormonal monitoring (β-hCG)

  • Exclusion of ectopic pregnancy or molar pregnancy

  • Discussion of options and patient preferences

  • Offering genetic testing of pregnancy tissue when indicated

Expectant Management

  • Waiting for the miscarriage to pass naturally

  • Suitable for stable patients with early pregnancy losses

  • Success rate 80% depending on gestation

Medical Management

  • Misoprostol ± mifepristone protocols

  • Allows management at home with medical support

  • High success rate (85–95%) for early pregnancy losses

Surgical Management

  • Suction curettage performed under sedation or anaesthesia 

  • Recommended for heavy bleeding, infection risk, patient preference, or failed medical management

  • Allows rapid resolution and collection of tissue for genetic analysis

  • Possible increased risk of intrauterine adhesions

  • Dr Rebecca Mackenzie-Proctor has access to surgical space if needing to undertake this procedure at short-notice.
